Under North Dakota Century Code, insurance companies are required to report fire losses to the State Fire Marshal. Companies can submit fire losses through this form.

Reports must include the following information:
- Name of the insured
- Date of the fire
- Amount of loss
- Loss paid
- Character of the property destroyed or damaged
- Supposed cause of the fire
Insurance companies should submit a report as soon as possible after notice of loss is received by the company. If any other reports are required to be submitted to the Insurance Commissioner, those reports must still be made. Submitting a fire loss to the Fire Marshal should be in addition to other legally required reports, not in lieu of.
